We are seeking a compassionate, experienced and motivated individual to work as a Aged Care Navigator within the Care Finder Program.
The Care Finder Program targets people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or other relevant supports in the community.
As an Aged Care Navigator, you will advocate for and support individuals so they are empowered to access appropriate resources, opportunities and aged care services within the community, primarily working with people living with HIV and LGBTIQ+ identifying people.
There is a full-time fixed term opportunity available until 30 June 2025, classified under the SCHADS Level 5. For all the details, including the position description, go to our Employment Page
