About Our Life+ HIV Support Program
The focus of the Life+ program is to support early initiation of treatment, encourage adherence to treatment and support People Living with HIV (PLHIV) to remain in care.
The Life+ program aims to reduce the time from diagnosis to treatment initiation thereby preventing HIV disease progression and reducing the possibility of onward transmission. We work with PLHIV to support treatment initiation, adherence and retention in care and help build the necessary skills and knowledge to effectively self-manage HIV.
The Life+ program improves HIV health literacy, address individual presentations of stigma and discrimination improve the overall quality of life for PLHIV.
LIFE+ is consists of 5 complementary pillars that provide integrated and complementary services for our community
Peer Navigation
Early intervention program to support those newly diagnosed with HIV and those re-engaging with care to navigate the complex environment of HIV care and treatment.
Case Management
A holistic approach to support PLHIV to address barriers such as homelessness, mental health and other issues that might impact their management of HIV.
Emergency Funding
Here at QPP, we believe that everyone has the right to safe and effective healthcare – this includes those who may not qualify for/under the Medicare system.
