About Case Management
Living well with HIV is more than just taking tablets. We all face challenges in life, and for people living with HIV it’s no different. QPP uses a holistic approach to support people living with HIV, through the LIFE+ program.
Our trained social and community work professionals work with PLHIV to address social determinant factors that impact remaining engaged in HIV treatment and care.
Our program can assist if you or someone you care for is having difficulties with managing issues such as:
- Homelessness
- Low mood or social isolation
- Substance misuse and addiction
- Managing HIV related illnesses
- Finances
- Transport
- Legal or immigration issues relating to HIV
We know these types of pressures can cause us to lose focus on our health and wellbeing including our HIV treatment and care. Our program is here to help – not to judge. The team of QPP Treatment Support Facilitators are trained social and community work professionals who work with people living with HIV to identify goals and develop action plans so PLHIV can improve their overall health and wellbeing.
Peer Navigation
Early intervention program to support those newly diagnosed with HIV and those re-engaging with care to navigate the complex environment of HIV care and treatment.
Emergency Funding
Here at QPP, we believe that everyone has the right to safe and effective healthcare – this includes those who may not qualify for support through the Medicare system.
